Central Cheshire Primary Care Mental Health Team

The Central Cheshire Primary Care Mental Health Team supports patients with mental health problems in the community and acts as a resource for GPs and other community staff. It is also a single point of access into secondary care mental health services.

The team provide treatment and support following assessment and can refer individuals to other appropriate services, including secondary care mental health services, community and third sector support and self-care models.
