Care Community Teams are a group of health and social care professionals who work alongside your local GP Practice to support you at home.
The team includes nurses, therapists and social workers. They also have strong links with many other health, local authority and voluntary agency teams.
Our Care Community Teams will work with anyone aged 18 or over at the time of referral.
To access this service, please speak to a GP, social worker, or any other health professional. They will be able to refer you if they think our service can help.
