Our Community Learning Disability Team are a health and social care team supporting adults who have a diagnosis of a learning disability.
The team supports people to lead full and healthy lives within the local community. They also provide specialist information, advice and training for family carers and support staff.
The community learning disability team work with anyone aged 18 and over who is living in West Cheshire.
To access our service, you can make a self-referral or a referral as a family member or carer by contacting us using the telephone number provided.
Alternatively, please speak to a GP, social worker, or any other health professional. They will be able to make a referral if they think our service can help.
